When providers document terms like rule out, probable, possible, or suspected, they may be clinically appropriate—but from a coding and auditing standpoint, those words carry significant implications. If not handled correctly, these phrases can result in inaccurate claims, failed medical necessity, or misleading diagnostic profiles that impact risk scoring and audit defense.

In this session, we’ll take a close look at how provisional diagnoses should be interpreted and reported, when they’re appropriate, and when they create problems. We’ll also address how to coach providers on alternative documentation strategies that preserve clinical integrity while supporting coding and compliance.

This isn’t just a coding issue—it’s about creating documentation that tells the whole story clearly, defensibly, and compliantly.

Evaluate the difference in coding approaches between inpatient and outpatient settings when provisional diagnoses are documented.
