Documentation must reflect a continuum of care, especially in primary care, geriatrics, and specialty practices where long-term management drives both quality and payment. This session explores how auditors can evaluate the record over time—not just for code accuracy, but for consistency, progression, and gaps in care documentation. You’ll learn how to identify red flags that signal poor continuity, and how to coach providers on reinforcing the clinical thread visit to visit.
