Transitional Care Management (TCM) is often misunderstood, underutilized, or—worse—misbilled. For physician educators, teaching TCM means more than just explaining timelines and phone calls. It requires clearly outlining the service intent, the documentation requirements, and how to operationalize this in a busy clinical setting. In this session, we’ll equip you with the tools to teach physicians the “why” and “how” of TCM in a way that resonates, reduces compliance risk, and improves capture of appropriate services. If you're responsible for provider education, this is a must-attend.
