Medical decision-making (MDM) hinges heavily on the complexity of the problems addressed—but what happens when documentation is vague, diagnoses are nonspecific, or the provider simply doesn't paint the full clinical picture? This session explores how incomplete or ambiguous documentation can obscure complexity and impact audit outcomes.

Auditors will learn to assess whether the documentation truly supports the level of service reported, how to identify gaps that diminish clinical complexity, and how to guide providers in closing those gaps. We’ll walk through common pitfalls and practical strategies for evaluating the intent and accuracy of diagnostic statements—highlighting the difference between coding to the highest specificity and auditing for clarity, completeness, and compliance.
