Modifier 25 remains one of the most misused—and most audited—modifiers in outpatient care. When diagnostic testing is performed alongside an E/M service, understanding when and how Modifier 25 applies is essential to ensure compliance and avoid denials. This session is designed specifically for those teaching physicians how to apply Modifier 25 correctly. We'll provide real examples, hands-on approaches, and practical teaching tips to help providers distinguish between services that are truly separate and those that are not. If your role includes physician education, this session will sharpen your messaging and help your providers code with confidence.
